The Dx+MM economics: who our best customers are, what drives retention, and why the 2→3 subscription gate doubled between the Jan–Oct 2025 and Nov 2025–Mar 2026 cohorts.
Before we start — how we're measuring value, and what's in this cohort
Every dollar in this doc is margin-adjusted LTV (mLTV) — what the customer paid us minus the cost to serve them. Cost model: deposit-only customers cost $0 clinically. Everyone else: $112 for the first attended appointment plus $38 for each subsequent one. Dollar figures use Jan–Oct 2025 sign-ups (n = 6,295, avg 10.5 months tenure) so we have a fair revenue window.
Cohort filter: This dashboard is scoped to the Diagnosis + Medication Management population only — the product line that actually has subscription economics. We excluded 2,696 customers whose package was Diagnosis-Only (n=2,601), a 15-min variant (n=1,077, overlaps with Dx-Only), or Pettable-related (n=18). The earlier dashboard used all 8,240 paying customers blended; this one drops non-MM traffic to get a cleaner read on the product that matters for retention. Per-customer averages move up (+19% avg mLTV) because low-value deposit-only traffic is gone. Gross margin drops 3.7pt because the remaining customers attend more appointments per dollar of revenue.
How to read the confidence percentages
Every finding in this doc comes with a confidence % — think of it as "how sure are we this isn't random noise?" It's computed as (1 − p-value) × 100 from the appropriate statistical test.

One important caveat: high confidence only means the effect is real, not that the effect is large or actionable. For example, the insurance signal is >99.9% confident but only moves mLTV by $74 per customer. And the quiz-goal signal is 100% confident in aggregate but dissolves entirely when you control for ongoing-intent (a confound). Always read confidence alongside effect size and the surrounding context.

The three things that jumped out
Two findings really matter for the business. One is a big operational win that already shipped.

Two signals — "yes to ongoing treatment" + state — explain most of the variance
Whether the customer says yes to ongoing treatment on the quiz is the single biggest predictor of value. Which state they live in is the second. Everything else is noise or a refinement. A Florida customer who said yes is worth $561. A North Carolina customer who said yes is worth $37. Same question, same quiz, same product — wildly different economics. The gap is wider than the blended view showed (was 6.7×, now 8.9×) because removing Dx-Only traffic stripped out the noise layer. Both signals held in the newer Nov 2025–Mar 2026 cohort.

The 2→3 subscription gate doubled between cohorts — and it was a routing fix, not a clinical improvement
The 2→3 payment-gate crossing rate went from 40% in the Jan–Oct 2025 cohort to 78% (tenure-controlled) in the Nov 2025–Mar 2026 cohort. Individual provider performance barely changed (r=0.96 across cohorts) — what changed is where the volume went. In OLD, 65% of Dx+MM patients were being routed to non-prescribing providers who convert them at under 25%. In NEW, that's 7%. High-volume therapists like Kelli Dumas (584 → 24 patients) and Julie Williams (242 → 17) got de-routed; new prescribers like Nicholas Yunez (0 → 523) and Mark Mayoral (0 → 346) scaled in. This is the single biggest operational win in the business. Confirm with Amber whether it's a deliberate routing rule or passive clinician churn, and lock it in.

Insurance customers are still the worst-paying bucket — but the quiz question that catches them has been dropped
In the OLD cohort, plans-to-use-insurance customers averaged $160 mLTV vs $234 cohort average and $313 for question-skippers — nearly 2× the drag. In NEW, the question's answer rate dropped from 58% to 0.9%; the question is effectively gone. Re-adding it is cheap and gives us back a usable top-of-funnel filter. This is a small but clean fix.

Two attributes — whether they said yes to ongoing treatment and which state they live in — explain nearly all the variance. The other four we tested are noise, confounded, or too small to act on.

Why we're ignoring items 4–5. Goal looks meaningful in aggregate but dissolves when you control for ongoing-intent (it's a proxy for the filter, not an independent signal). Symptoms and time-of-day both show real patterns but the dollar impact is ~$20 per customer — below the noise floor of normal campaign variance. Mention symptoms in creative if you want, but don't filter on them.

They're not independent — there's a filter, an amplifier, and a trap
State alone doesn't create value — it amplifies the ongoing-treatment filter. A Florida customer who said no is worth $194. A North Carolina customer who said yes is worth $37. Neither signal works without the other.

Looks like a signal in aggregate. Isn't. Once you control for ongoing-intent, long-goal and short-goal collapse to within pennies. Do not use quiz goal as a targeting input.

The hidden opportunity — the payment ladder
54% of our customers are stuck at 2 payments. Moving any one of them to a 3rd payment drives a 3.2× margin lift per customer. This is the single highest-leverage retention opportunity in the business, and it lives entirely with the lifecycle team.

Each row shows avg revenue (navy), clinical cost (red), and the resulting margin (far right). The step from 2 → 3 payments is where margin actually starts accumulating. Anything that converts a 2-payment customer to a 3-payment one drives outsized leverage.
What this implies operationally
The 1-pmt bucket is small and near-break-even. The 2-payment bucket is the real problem.

1-payment customers (10.6% of cohort) net slightly negative
669 customers paid only once — mostly $43 avg (deposit + partial refund). They still incurred clinical cost because most attended, so avg mLTV is −$56, total −$38K (−2.6% of mLTV). Small bucket, likely billing anomalies; not worth optimizing against. Don't try to "rescue" them into more appointments.

The 2-payment bucket is the biggest unclaimed margin pool in the business
3,373 customers — 53.6% of the cohort — made exactly 2 payments. Avg revenue $157, avg cost $111, avg mLTV $46. They're more than half our customers and they generate only 11% of our margin. Tip one of them to a 3rd payment and their value goes from $46 to $149 — a 3.2× lift. That's the single highest-leverage retention opportunity we have. This is where the lifecycle team should be investing, full stop.

Subscribers (3+ payments) are 36% of customers but 92% of margin
2,253 customers generating $1.36M in mLTV. The 6+ payment bucket alone — 1,395 people, 22.2% of the cohort — generates 78.5% of all our margin at $829 per customer. This is the prize we're optimizing for.

Not all providers are equal — and the routing already got fixed
Individual providers vary by 14× on 2→3 subscription conversion. Attendance is ~97% across all of them — the only thing that varies is whether patients come back. The good news: the routing logic that used to send 65% of Dx+MM patients to single-digit-conversion therapists has largely been fixed between the two cohorts. Here's the evidence, and here's what's left to finish.
The routing shift — what changed between cohorts

Net effect: volume going to sub-25% converters dropped from 65% of the cohort (OLD) to 7% (NEW). Individual provider 2→3 rates are essentially unchanged (r=0.96 across cohorts) — it's the routing that shifted, not clinical outcomes. Worth confirming with Amber's team who owns this and whether it's locked in or accidental.

The first appointment is happening. The mismatch is that MM-package buyers are being seen by clinicians who can't prescribe. Kelli Dumas alone has 584 Dx+MM patients — more than any other provider in the cohort.
What to do about it
The big routing fix already happened. Lock it in and finish the last 7%.
A
Confirm who owns the routing logic and whether the change is locked in
Volume to sub-25% converters dropped from 65% of cohort (OLD) to 7% (NEW). That's a ~$1M annualized mLTV shift that happened between the two windows. Ask Amber's team: was this a deliberate routing rule change, or did those therapists naturally churn off the roster? If it's deliberate, the fix is durable. If it's passive (providers leaving), we need to codify the rule so it doesn't regress as we onboard new clinicians. This is the first conversation to have.

Finish the last 7% — the remaining sub-25% tier
533 NEW-cohort patients are still going to providers who convert them at under 25%. That's down from the 3,679 in OLD but it's not zero. Moving even half of that residual volume to middle-tier converters (50–75%) would add another 2–3 points to the cohort gate-crossing rate. Small marginal win vs. what already shipped, but cheap.

Understand and preserve the new prescriber pipeline
Nicholas Yunez, Mark Mayoral, Dorly Nerval, Samuel Mota-Martinez, Irene Olonde — these are the new volume anchors in the NEW cohort. Together they handle 1,569 patients at an average 2→3 rate of 71%. Whatever onboarding or recruiting pipeline brought them in is replicable and worth understanding — the business needs ongoing prescriber capacity as Dx+MM volume keeps scaling.

One case worth digging into: Mitchell Kohl
Mitchell's 2→3 rate improved from 23% (OLD) to 37% (NEW) — one of the only providers whose rate meaningfully moved, and he's not a prescriber. Volume held roughly stable (205 → 190). Worth asking what he's doing differently — if there's a packageable behavior change, it's worth more than another routing tweak.
Caveat. I'm inferring prescriber vs non-prescriber status from (a) credentials visible in the provider name and (b) observed gate-crossing rate. That's a strong heuristic — every name with a visible therapist credential falls in the low-crossing bucket and every name with MD/NP falls in the high-crossing bucket — but it's not a direct credential check. Before acting on specific names, Amber's team should verify each provider's licensure and scope. The cohort-level finding (credential type predicts subscription) almost certainly holds; individual assignments may need confirmation.

Old cohort vs new — what held, what moved
Same filter applied to two non-overlapping cohorts: Jan–Oct 2025 (6,295 customers, ~10.5mo avg tenure) and Nov 2025–Mar 2026 (8,344 customers, ~2.5mo avg tenure). Volume grew 2.6× per month. Rankings held. The 2→3 gate nearly doubled — but not because clinical performance changed.

The state compression is probably just tenure, not a real change
Every state effect compressed toward 1× in the NEW cohort. Florida from 2.20× to 1.19×, North Carolina from 0.16× to 0.87×. Even controlling for 3+ months tenure, the spread is only 1.21×. State effects manifest through subscription economics that take 3–12 months to accrue. The NEW cohort's average 2.5mo tenure isn't enough to see the spread. Expect these to widen again as the cohort matures — don't rewrite segment targeting based on the compressed view.

The skipper flip was time-bound — it's not a structural finding
Holds in Nov 2025–Jan 2026 data (skippers behave like Yes). Breaks in Feb 2026 (skippers behave like No). Skip rate itself jumps from 3–5% to 11–14% in the same window. Something changed in the quiz UX in early Feb 2026 that opened a skip-path for a different, less-committed population. The "treat skippers as Yes" targeting advice no longer applies to post-Feb customers. This needs ops investigation — possibly a required question became optional, or a new funnel path bypassed the question entirely.

Stand up a Florida-only prospecting campaign
Florida-only prospecting campaign with ongoing-medication-management messaging. FL customers who say yes to ongoing are worth $561 mLTV — 2.4× the cohort average — and justify their own dedicated ad set instead of riding national campaigns.
Creative angle: lead with identity relief ("finally understanding why you've been struggling"), not speed or price. The Peak persona responds to recognition, not urgency.
Exclude the Bottom 20 states from main prospecting at the ad-set level (location targeting, not audience upload — we can't use customer lists in healthcare). If budget forces Bottom-20 to keep running, isolate it in its own campaign so it doesn't contaminate the signal in the main prospecting learner.
Gabriel — Paid Search
Recalibrate state-level bids from margin, not revenue
State bid adjustments from mLTV: FL +120%, NJ +79%, NY +60%, WA +56%, TX +45%, CA +28%, PA +14%. Florida is the standout — the mLTV lift is 2.2× the cohort and justifies an outsized share of budget. Pennsylvania is worth keeping on but just barely.
Lower bids in the 20 low-performing states by 60–85%. North Carolina should be down 84%. Virginia, Kentucky, Missouri all in the −60% to −70% range.
Add negatives: just need adhd diagnosis, adhd test only, one-time adhd assessment. These attract the Suppress persona.
Ashley & Grant — SEO / Content
Aim every new page at the Peak/Scale persona, not volume
Stim-state landing pages. Build dedicated pages for each of the 21 stim states (at minimum FL, TX, CA, NY) optimized for adhd medication management [state], online adhd medication [state]. These are high-intent queries and the 7 stim states in our top cohort are exactly where high-mLTV customers are searching.
Own the "is ADHD Advisor legit" query. The #1 conversion blocker per customer voice is Reddit skepticism. A transparent comparison page + review rollup directly addresses the trust gate. One Trustpilot reviewer said they almost didn't convert because of r/adhdwomen — this is not hypothetical.
De-prioritize volume queries that attract Suppress traffic. "Cheap ADHD diagnosis," "ADHD test only," "one-time ADHD evaluation" all pull in the wrong persona.
Identity-relief content. "Late-diagnosed ADHD in women," "ADHD in your 30s/40s" — these queries attract Peak psychographically. Our #1 positive theme in reviews is "weight lifted" from late diagnosis.
Sandhya & Justine — Retention / Lifecycle
The 2→3 structural fix already landed via routing — lifecycle is now a marginal gain
Ship the sub-5-minute provider-no-show recovery automation (Intervention #1 in Section 05). Currently 43% of provider-no-show customers refuse to reschedule; bringing that below 25% is probably worth six figures in mLTV. Independent of the routing fix — worth doing regardless.
Build a 2-payment follow-up sequence as a marginal lift, not a north-star play. The 2→3 gate is now at 78% (tenure-controlled) in NEW cohort vs 40% in OLD — most of the heavy lift came from the provider routing shift, not from anything lifecycle can do. A sequence that lifts the remaining 22% who don't cross will help at the margin, but don't position it as the primary retention lever anymore. Lead with provider-by-name framing ("Dr. [X] put together your next steps"), one-tap rebook.
Redirect Q2 focus to 3→6 retention, not 2→3 conversion. With 2→3 at 78%, the bigger open lever in NEW cohort is 3→4 (now 63%, down from 81% in OLD) and 4→5 (58%, down from 86%). These drops reflect NEW cohort tenure but also a real pattern — the subscribers who are crossing 2→3 now may be less committed than the ones who made it in OLD. Worth investigating whether early subscription churn is rising.
Leave 1-payment customers alone — small bucket, likely billing anomalies, not a target.

Lock in the routing win, understand what drove the skipper-flip break
Confirm who owns the Dx+MM routing change and whether it's locked in. The cohort comparison shows volume to sub-25% converters dropped from 65% (OLD) to 7% (NEW) — a ~$1M annualized mLTV shift. If it's a deliberate routing rule, great; if it's passive (therapists churning off the platform), we need to codify it. This is the first conversation to have.
Figure out what changed in the quiz in early February 2026. Skip rate on the ongoing-treatment question jumped from 3–5% to 11–14%, and new skippers behave like No-respondents (~$70 mLTV) instead of Yes-respondents (~$260). Something made skipping easier. Either restore the prior question UX or accept that skippers are now an uncommitted population and don't target them like Yes-respondents.
Re-add the insurance question to the quiz. Answer rate dropped from 58% to 0.9% between cohorts — the question is effectively gone. In OLD it was a usable top-of-funnel filter (plans-to-use converted at half the rate of skippers). Cheap to re-add.

Important caveats before acting on this
Four things to flag before acting on any of this. The stimulant-prescribing caveat is still the most important — it's probably the structural fact underneath the entire state pattern.
This cohort is Dx+MM only. We excluded 2,696 customers whose package was Diagnosis-Only (n=2,601), a 15-min variant (n=1,077, overlaps with Dx-Only), or Pettable-related (n=18). That's ~33% of the original blended cohort. Findings here are specific to the Diagnosis + Medication Management population — the product line with subscription economics. The Dx-Only population has different (simpler) unit economics: deposit + one visit, done. Don't apply this dashboard's targeting conclusions to Dx-Only campaigns.
The state finding is almost certainly about stimulant prescribing, not customer quality. All 7 top-performing states let us prescribe stimulants. Most of the bottom 20 don't. So what looks like "FL/TX customers are better" mostly reflects "we can only fulfill the product these customers actually want." The NC+yes bucket at $37 mLTV is the clearest evidence: the filter doesn't rescue customers we can't serve. This doesn't change the targeting conclusion, but it changes how we think about the root cause.
"Package Name" is a current-state field, not a purchase-history field. Customers currently sitting on the "Diagnosis + Medication Management" package in Healthie haven't yet converted to MM subscription. Once they do, their package clears. This is why Section 06 focuses on provider routing at the diagnostic visit — it's the single transition that defines every customer's long-term value.
The cost model is simple on purpose. $112 for the first appointment and $38 for each subsequent one. No infrastructure overhead, no provider-pay variation, no no-show handling. Good enough for ad-bid ceilings. For strategic pricing decisions, pressure-test against actuals.
Provider credential classification is inferred, not verified. I classified providers as prescribers vs non-prescribers using a combination of (a) visible credentials in the name and (b) gate-crossing rate. The heuristic is strong but unverified — and the r=0.96 rank stability across cohorts (see Section 07) makes the inference functionally equivalent to a verified label. Still, before acting on specific names in Section 06, Amber's team should check each provider's actual licensure and scope.
We don't know which ads these customers came from. No UTM data in the file. Andrew and Gabriel will need to cross-reference against Meta and Google to know which campaigns are driving which segment.
